- Optional email report delivery - You may request a copy of your Readiness Report sent to your email. Entirely optional.
- Optional advocate or representative contact - You may separately and explicitly opt in to have your information shared with a participating disability advocate or representative. By submitting the screening, you do not automatically consent to advocate contact. Only if you affirmatively check this option will your information be shared. This has no effect on your Readiness Report.
- Optional contact preferences (phone, text, email) - If and only if you opt in to advocate contact, you may indicate your preferred contact methods. These preferences are not activated unless you have opted in to advocate contact.
You may withdraw any optional consent at any time by contacting us. Participating advocates and representatives are required to maintain confidentiality and comply with applicable laws.
13. Limitation on Screening
Our screening tool:
- Is NOT a legal determination of eligibility
- Does NOT guarantee you will qualify for benefits
- Is NOT a substitute for professional legal advice
- Is based on publicly available SSA criteria
- May not account for all factors SSA considers
