  • Denial notice or SSA letters, if any
  • Application status and confirmation details
  • Appeal deadline, if any is listed on your notice
  • Medical conditions and diagnoses
  • Doctors, clinics, and treatment providers
  • Recent treatment dates and types of treatment
  • Medication list and side effects
  • Work history and last-worked date
  • Daily limitations and how your condition affects activities
  • Current earnings, if you are still working
  • Existing representative or advocate information, if any
