What the Screening Collects

Our readiness screening is a free, informational tool for individuals considering disability benefits. With the user's permission, the intake may collect the following types of information:

  • Claim stage (first application, appeal, hearing, etc.)
  • State of residence
  • Age range
  • Work status and last-worked details
  • Medical condition and duration
  • Treating doctor and recent care details
  • Functional limitation details (sitting, standing, lifting, cognitive, etc.)
  • Documentation gaps identified by the screening
  • Optional advocate or representative contact consent (separate, explicit, and never automatic)

What Partners May Receive

With the user's separate permission, participating advocates or representatives may receive a structured summary that can help determine whether they want to contact the user about possible next steps.

  • Information is only shared when the user explicitly checks the optional advocate contact consent box during the screening.
  • Not every user will request advocate or representative contact.
  • Partners do not receive leads automatically - routing is consent-gated.
  • A participating advocate or representative may contact the user - this is not a guarantee of contact, case review, or representation.

What This Is Not

  • Not a guarantee of signed cases or case acceptance
  • Not a guarantee that a user is eligible for disability benefits
  • Not a promise that every user will request advocate or representative contact
  • Not a representative referral service unless separately structured and compliant with applicable rules
  • Not a substitute for your own intake review and professional judgment
  • Not a ranking or recommendation of any advocate or representative to users
