Advocate Contact Disclosure

Your information will not be shared with participating advocates or representatives without your separate, explicit consent.

At the end of the readiness screening, you will be given a separate, clearly labeled option to request contact from a participating disability advocate or representative. This is entirely optional and has no effect on your Readiness Report. Your information will only be shared if you affirmatively check that box.

Participating advocates and representatives are required to maintain the confidentiality of your information and use it only to evaluate your potential disability claim.

Contact Consent

At the end of the screening, you will be presented with four clearly labeled consent options:

  • Required acknowledgment: You must confirm that this tool is for informational use only, is not affiliated with the SSA, and does not guarantee eligibility. This is required to generate your Readiness Report.
  • Optional email report delivery: You may request a copy of your Readiness Report sent to your email address. This is entirely optional.
  • Optional advocate or representative contact: You may separately and explicitly opt in to have your information shared with a participating disability advocate or representative. This is entirely optional and has no effect on your Readiness Report.
  • Optional contact preferences (phone, text, email): If and only if you opt in to advocate contact, you may indicate your preferred contact methods. These preferences are only used for advocate or representative contact and are not activated unless you have opted in to advocate contact.

You can withdraw any optional consent at any time by contacting us. Your consent to advocate contact is not a condition of using our screening tool or receiving your Readiness Report.
