Disclosures

Important information you need to know

Not Affiliated with the SSA

This website is NOT affiliated with, endorsed by, or associated with the Social Security Administration (SSA). We are a third-party screening tool operated independently. The SSA does not endorse or recommend our service.

Not Legal Advice

This website does not provide legal advice. We are not legal professionals, and nothing on this website should be construed as legal advice or the creation of a representative relationship.

If you need legal advice about your disability claim, you should consult with a qualified disability advocate or representative licensed to practice in your state.

No Guarantee of Eligibility

We do NOT guarantee that you will qualify for or receive disability benefits. Our screening tool provides an assessment based on information you provide and publicly available SSA criteria.

Only the Social Security Administration can make final eligibility determinations. Your actual eligibility depends on many factors, including a complete medical examination, work history verification, and SSA's evaluation of your specific circumstances.

Final Decisions Made by SSA Only

The Social Security Administration is the sole authority for determining disability eligibility. Our screening tool is informational only and cannot:

  • Determine your actual eligibility for benefits
  • Predict the outcome of your claim
  • Replace the official SSA application process
  • Substitute for a complete medical evaluation
  • Account for all factors SSA considers

If you believe you may qualify for disability benefits, you should apply directly with the SSA or work with a qualified disability representative.

Optional Advocate Contact

Your information will not be shared with participating advocates or representatives without your separate, explicit consent.

How Advocate Contact Works

At the end of the readiness screening, you will be given a separate, clearly labeled option to request contact from a participating disability advocate or representative. This is entirely optional and has no effect on your Readiness Report.

What Information May Be Shared (Only With Your Consent)

If and only if you affirmatively opt in, the following may be shared with a participating advocate or representative:

  • Your contact information (name, email, phone)
  • Your readiness summary
  • Your medical condition information
  • Your work history and income information
  • Your functional capacity information

When Information Is Shared

Your information is shared only when you have separately and explicitly opted in to advocate contact at the end of the screening form.

Participating Advocates

Participating advocates are required to:

  • Maintain confidentiality of your information
  • Use your information only to evaluate your potential claim
  • Comply with all applicable laws and regulations
  • Respect your contact preferences

Opting Out

You can opt out of lead sharing at any time by contacting us. You can also decline specific contact methods (phone, text, email) during the screening process.

Consent Model

At the end of the screening, you will be presented with four clearly labeled consent options. Only the first is required to generate your Readiness Report.

1. Required Informational Acknowledgment

You must confirm that this tool is for informational preparation only, is not affiliated with the SSA, is not legal advice, and does not guarantee eligibility. This acknowledgment is required to generate your Readiness Report.

2. Optional Email Report Delivery

You may optionally request a copy of your Readiness Report sent to the email address you provided. This is entirely optional and has no effect on your report.

3. Optional Advocate or Representative Contact

You may separately and explicitly opt in to have your information shared with a participating disability advocate or representative. This is entirely optional. Your information will not be shared unless you affirmatively check this option. This has no effect on your Readiness Report.

4. Optional Contact Preferences (Advocate Contact Only)

If and only if you opt in to advocate contact, you may indicate your preferred contact methods: phone calls, text messages, and/or email. These preferences are only used for advocate or representative contact and are not activated unless you have opted in to advocate contact. Message and data rates may apply for text messages.

Withdrawing Optional Consent

You can withdraw any optional consent at any time by contacting us. You may also:

  • Reply "STOP" to any text message to stop texts
  • Click "unsubscribe" in any email to stop emails
  • Email us to withdraw all optional contact consent

Screening Tool Limitations

Our screening tool has the following limitations:

  • Based on information you provide (accuracy depends on your responses)
  • Uses publicly available SSA criteria (may not include all factors SSA considers)
  • Does not include a medical examination (SSA requires this)
  • Does not verify work history or income (SSA will verify this)
  • Does not account for state-specific variations in SSI eligibility
  • Does not consider your specific medical records (SSA will review these)
  • Is not updated in real-time (SSA criteria may change)

Data Security

We use industry-standard security measures to protect your information. However:

  • No method of transmission over the Internet is 100% secure
  • We cannot guarantee absolute security of your information
  • You use our website at your own risk
