How to Organize Doctor and Treatment Information

Organizing your doctors, clinics, treatment dates, medications, and side effects into one place may make it easier to prepare for a disability benefits application, appeal, hearing, or consultation with an advocate or representative.

Doctor and Provider List

For each doctor, clinic, or treatment provider, consider recording the following.

  • Doctor or provider name
  • Clinic or hospital name
  • Specialty (primary care, cardiologist, psychiatrist, etc.)
  • Phone number
  • Address or patient portal information
  • Date of first visit
  • Date of most recent visit
  • Purpose of treatment

Treatment Timeline

Listing treatment events in order may help you see gaps or missing records. Consider including the following events with approximate dates.

  • Diagnosis date, if known
  • Major tests (imaging, lab work, specialist evaluations)
  • Specialist visits
  • Hospital or emergency room visits
  • Surgeries or procedures
  • Therapy or counseling (physical, occupational, mental health)
  • Significant medication changes

Medication and Side Effects List

For each medication, consider recording the following.

  • Medication name
  • Dosage
  • Prescribing provider
  • Start date, if known
  • Side effects experienced
  • Whether the medication helps, causes problems, or both

What May Be Missing

After organizing your information, consider whether any of the following may be missing.

  • Recent records from the past 12 months
  • Specialist records for your primary condition
  • Test results, imaging, or lab work
  • Records covering treatment gaps
  • Side-effect details for current medications
  • Assistive device information (cane, walker, brace, oxygen, etc.)
