# Purpose

The free readiness screening asks guided questions to help a person organize information that may be important when preparing to apply, appeal, or speak with an advocate or representative.

# Information Collected

The screening may ask about:

* age range;
* recent work history and current work status;
* physical or mental conditions;
* how long the condition has affected work;
* treatment, doctors, medication, and medical records;
* daily and work-related limitations;
* application, denial, or appeal status;
* whether the person already has an advocate or representative; and
* contact information and separately presented consent choices.

# Output

The screening can produce a personalized [Readiness Report](/process/readiness-report.md) that summarizes the person's answers, identifies possible information gaps, and suggests preparation steps.

# What It Does Not Do

The screening does not:

* submit an application to the Social Security Administration;
* access an official Social Security record;
* make an eligibility determination;
* predict the chance of approval;
* provide a medical diagnosis; or
* provide legal advice.
